Agreement and Disclosures

I understand that if I enroll my checking, savings, certificate of deposit, IRA or loan account(s) in the eStatement service I will receive all account statements and notices related to that account via e-mail. Once enrolled, I will no longer receive paper account statements or notices (such as privacy notice, NSF notice, loan billing notice, IRA and COD (certificate of deposit) approaching renewal or maturity notice, change in terms notice, etc.), via US mail.

I understand that a per page printing fee may apply if I request additional paper copies of my statements and notices from the bank.

I understand and agree to submit my e-mail address to United Community Bank as a part of this application.

I understand that if I change my e-mail address, I must notify United Community Bank in writing or via e-mail of my new e-mail address. If I fail to notify United Community Bank of my new e-mail address, I understand that I may not receive my statements or notices that are e-mailed to me.

I agree that I have the necessary software installed to receive my statement and other documents electronically via e-mail.

I understand that my Personal Identification Number (PIN) will be generated automatically by the Electronic Statement System. My PIN will contain the first five characters of my email address and the last four digits of the primary account holder's Social Security Number or Tax ID. For example the PIN for abcde1111@email.com with a Social Security Number of 123-45-6789 would be abcde6789. E-mail addresses are all listed in lower case letters.

I understand that the security of my PIN is my responsibility and that anyone who has access to my PIN will be able to view my e-mail statement and other notices. I further understand and agree to notify United Community Bank immediately if my PIN should become compromised or known to individuals who are not authorized by me.

Only the statements and notices for the account(s) listed in this application will be e-mailed to the address listed in this application. The statements of other United Community Bank customers cannot be sent to the same e-mail address listed in this application.

I understand that United Community Bank reserves the right to amend this disclosure (agreement) in any manner and at any time. If United Community Bank does so, the bank will give me at least ten (10) days advance written notice. The bank also reserves the right to terminate this agreement by sending a notice of such termination by first-class mail to my last known address on the bank's records.

I understand that I may withdraw my consent to receive my statement and other documents electronically by written notice to United Community Banks, Inc., Attn: Online Banking Support, P O Box 398, Blairsville, GA 30514 or send an e-mail message to the bank at onlinebanking@ucbi.com. I understand that the bank must receive the notice at least ten (10) days before the statement is to be e-mailed to my current e-mail address.
